Oak Mountain Outdoor Healthcare (OMOHC) is building a selective, interdisciplinary provider network committed to environment-first, value-based care that integrates clinical excellence, nature-based interventions, and outcomes-driven accountability.
We partner with licensed clinicians, researchers, and allied health professionals who are interested in participating in pilot programs, CMS/CMMI-aligned demonstrations, and scalable care models that address chronic disease, behavioral health, and health equity through evidence-informed outdoor and environmental health strategies.

Accepted providers may participate in:
OMOHC pilot programs and intensive outpatient models
CMS/CMMI-aligned value-based care demonstrations
Interdisciplinary care teams integrating behavioral, medical, and environmental health
Standardized outcomes tracking, including functional, metabolic, behavioral, and quality-of-life metrics
Research-informed practice, with opportunities for publication and program evaluation
Ethical, trauma-informed, and equity-centered care delivery
Participation does not require exclusive affiliation and does not constitute employment unless otherwise specified.
Provider Expectations
All network providers agree to:
Maintain active licensure and professional liability coverage
Practice within scope of license and evidence-based standards
Participate in outcomes measurement and quality improvement
Align with OMOHC’s environment-first, patient-centered philosophy
Comply with HIPAA, ethical guidelines, and applicable regulations
Engage respectfully in interdisciplinary collaboration
